The good news: it's free, the requirements are simple, and — contrary to a widespread myth — you don't need a NISS to get it. What the Número de Utente Is
The número de utente is your unique identifier in the SNS. With it, you can register at a centro de saúde (health centre), be assigned a family doctor where available, get prescriptions, and use public hospitals. It's the key that turns "I live in Portugal" into "I can use the health system here."
Who Can Register
Any legal resident can obtain a número de utente:
- A Portuguese national, or
- A foreign national holding a residence permit (autorização de residência).
The número de utente gives that resident access to the public health service. In other words, once you hold a valid residence permit, healthcare access is your right — you just need to complete the registration.
The Documents You Need
For a foreign national, bring:
- Your residence permit (autorização de residência) — this supplies the number and its validity.

That's the core set. The número de utente is granted on the basis of the residence permit (with the NIF) — not on your employment or social-security status.
The Myth: "You Need a NISS First"
This one causes real confusion, so let's be clear: a NISS is not required to register for the SNS. The número de utente is based on your residence permit and NIF, not your NISS (the social-security number). Retirees who don't work in Portugal generally don't need a NISS for healthcare at all.
The NISS is a work and contributions number — you need it to take up employment or register as self-employed, not to see a doctor. Register at the centro de saúde of the freguesia (parish) where you live. The número de utente is assigned when you first attend a public health unit — your local centro de saúde, or a hospital — presenting the required documents.

Since 1 June 2022, taxas moderadoras (user fees) were abolished across almost all SNS services — health-centre consultations, prescribed exams and analyses, and first and follow-up hospital consultations are free. A fee remains only for non-referred hospital emergency (urgência) attendances.
So the running cost of using the public system is far lower than many newcomers expect.

What if you've only just arrived and don't yet have permanent residence? There's a route for that.
